De precieze oorzaak van tropische spruw, ook wel postinfectieuze tropische malabsorptie is niet bekend. Er wordt van uit gegaan dat de aandoening het gevolg is van een infectie die wordt veroorzaakt door parasieten of bacteriën. Welke ziekteverwekker dat is, is niet bekend. Het syndroom bestaat uit morfologische afwijkingen in de dunne darm die samengaan met malabsorptie.
