General comments
Onderhoudsdosering vancomycine afhankelijk van gemeten spiegels en van nierfunctie, zie bij vancomycine en bij TDM vancomycine
E. faecalis is meestal amoxicilline-gevoelig, E. faecium is gewoonlijk amoxicilline-ongevoelig.
Bij gebruik van vancomycine moeten spiegelbepalingen uitgevoerd worden.
Denk bij intraveneuze antibiotische therapie >3 dagen aan de mogelijkheid van thuisbehandeling (zie iPortal protocol: OPAT, antibiotica infuustherapie voor thuis).
